Physical Activity Readiness Questionnaire (PAR-Q)

If you are enrolling on any course, workshop or class with Rainbow Circus CIC, you must complete one of the below forms.
Each participant is required to have a completed Student Par-Q before commencing the class. 
Personal details will be held in accordance with the Data Protection Act and will only be used to contact you regarding Rainbow Circus CIC or in an emergency. Data can be destroyed on your request within 4 weeks of the request if it is appropriate to do so.
